第一節(jié)   施工現(xiàn)場(chǎng)質(zhì)量管理檢查記錄表
該表是GB50300—2001第3.0.1條的附表，健全的質(zhì)量管理體系的具體要求。一般一個(gè)標(biāo)段或一個(gè)單位（子單位）工程檢查一次，在開(kāi)工前檢查，由施工單位現(xiàn)場(chǎng)負(fù)責(zé)人填寫(xiě)，由監(jiān)理單位的總監(jiān)理工程師（建設(shè)單位項(xiàng)目負(fù)責(zé)人）
　　驗(yàn)收。下面分三個(gè)部分來(lái)說(shuō)明填表要求和填寫(xiě)方法。
　　一、表頭部分填寫(xiě)參與工程建設(shè)各方責(zé)任主體的概況。由施工單位的現(xiàn)場(chǎng)負(fù)責(zé)人填寫(xiě)。
　　工程名稱欄。應(yīng)填寫(xiě)工程名稱的全稱，與合同或招投標(biāo)文件中的工程名稱一致。
　　施工許可證（開(kāi)工證），填寫(xiě)當(dāng)?shù)亟ㄔO(shè)行政主管部門批準(zhǔn)發(fā)給的施工許可證（開(kāi)工證）的編號(hào)。
　　建設(shè)單位欄填寫(xiě)合同文件中的甲方，單位名稱也應(yīng)寫(xiě)全稱，與合同簽章上的單位名稱相同。建設(shè)單位項(xiàng)目負(fù)責(zé)人欄，應(yīng)填合同書(shū)上簽字人或簽字人以文字形式委托的代表——工程的項(xiàng)目負(fù)責(zé)人。工程完工后竣工驗(yàn)收備案表中的單位項(xiàng)目負(fù)責(zé)人應(yīng)與此一致。
　　設(shè)計(jì)單位欄填寫(xiě)設(shè)計(jì)合同中簽章單位的名稱，其全稱應(yīng)與印章上的名稱一致。設(shè)計(jì)單位的項(xiàng)目負(fù)責(zé)人欄，應(yīng)是設(shè)計(jì)合同書(shū)簽字人或簽字人以文字形式委托的該項(xiàng)目負(fù)責(zé)人，工程完工后竣工驗(yàn)收備案表中的單位項(xiàng)目負(fù)責(zé)人也應(yīng)與此一致。
　　監(jiān)理單位欄填寫(xiě)單位全稱，應(yīng)與合同或協(xié)議書(shū)中的名稱一致。總監(jiān)理工程師欄應(yīng)是合同或協(xié)議書(shū)中明確的項(xiàng)目監(jiān)理負(fù)責(zé)人，也可以是監(jiān)理單位以文件形式明確的該項(xiàng)目監(jiān)理負(fù)責(zé)人，必須有監(jiān)理工程師任職資格證書(shū)，專業(yè)要對(duì)口。
　　施工單位欄填寫(xiě)施工合同中簽章單位的全稱，與簽章上的名稱一致。項(xiàng)目經(jīng)理欄、項(xiàng)目技術(shù)負(fù)責(zé)人欄與合同中明確的項(xiàng)目經(jīng)理、項(xiàng)目技術(shù)負(fù)責(zé)人一致。
　　表頭部分可統(tǒng)一填寫(xiě)，不需具體人員簽名，只是明確了負(fù)責(zé)人的地位。
　　二、檢查項(xiàng)目部分填寫(xiě)各項(xiàng)檢查項(xiàng)目文件的名稱或編號(hào)，并將文件（復(fù)印件或原件）附在表的后面供檢查，檢查后應(yīng)將文件歸還。
　　1、現(xiàn)場(chǎng)質(zhì)量管理制度。主要是圖紙會(huì)審、設(shè)計(jì)交底、技術(shù)交底、施工組織設(shè)計(jì)編制審批程序、工序交接、質(zhì)量檢查評(píng)定制度，質(zhì)量好的獎(jiǎng)勵(lì)及達(dá)不到質(zhì)量要求處罰辦法，以及質(zhì)量例會(huì)制度及質(zhì)量問(wèn)題處理制度等。
　　2、質(zhì)量責(zé)任制欄，質(zhì)量負(fù)責(zé)人的分工，各項(xiàng)質(zhì)量責(zé)任的落實(shí)規(guī)定，定期檢查及有關(guān)人員獎(jiǎng)罰制度等。
　　3、主要專業(yè)工種操作上崗證書(shū)欄。測(cè)量工、起重、塔吊垂直運(yùn)輸司機(jī)，鋼筋、混凝土、機(jī)械、焊接、瓦工、防水工等建筑結(jié)構(gòu)工種。
　　電工、管道等安裝工種的上崗證，以當(dāng)?shù)亟ㄔO(shè)行政主管部門的
